In accordance with the Personal Data Protection Law No. 6698 (“KVKK”), this clarification text has been prepared to inform you about the purposes of processing your personal data, to whom it may be transferred, the methods of collection, and your rights.

1. Data Controller
   Your personal data may be processed by Op. Dr. Ali Sipahier, in his capacity as the data controller, within the framework explained below.

   The following personal data will be processed during your examination and treatment process:

• Identification information (Name, surname, T.C. identity number, etc.)

• Contact information (Phone, email, address, etc.)

• Health data (Examination details, diagnosis, treatment, medical images, prescriptions, etc.)

• Appointment and payment information

• Camera recordings (for clinic security purposes)

3. Purposes of Processing Personal Data
   Your collected personal data will be processed for the following purposes:

• Provision and follow-up of health services

• Conducting medical diagnosis, treatment, and care services

• Managing appointment procedures

• Fulfillment of legal obligations

• Billing and payment processing

• Ensuring clinic security

4. Transfer of Personal Data
   Your personal data may be shared within the legal framework specified in Articles 8 and 9 of the KVKK with the following parties:

• Ministry of Health and its affiliated institutions

• SGK and other relevant public institutions

• Authorized judicial and administrative authorities

• Legal service providers in case of legal obligations

• Suppliers, accounting, and consulting firms with whom the clinic has a business relationship

5. Methods of Collecting Personal Data and Legal Basis
   Your personal data is collected directly from you through examination forms, patient registration systems, digital applications, and camera systems.

Your data is processed based on the legal grounds stated in Articles 5/2 (a), (c), (ç) and 6/3 of the KVKK, as follows:

• Explicitly provided for in laws

• Directly related to the establishment or performance of a contract

• For the data controller to fulfill its legal obligations

• Necessary for the provision of health and medical services

6. Your Rights as a Data Subject
   Under Article 11 of the KVKK, you have the following rights:

• To learn whether your personal data is being processed

• To request information about it if processed

• To learn the purpose of processing and whether it is used in accordance with the purpose

• To know the third parties to whom your data is transferred, both domestically and internationally

• To request correction if the data is incomplete or incorrect

• To request the deletion or destruction of your data if processed in violation of the KVKK

• To request notification of these actions to third parties to whom the data has been transferred

• To object to the processing of data exclusively by automatic systems that result in unfavorable outcomes for you

• To request compensation for damages caused by unlawful processing of your personal data
